Volunteer/Pet Orientation

on the Adelup Grounds (Govenor's Complex) during the mini fair. Just bring yourself and your dog!

- Download Pet Health Evaluation form Here
- Download Volunteer Pledge Here Read and Sign
- Download Oath of Confidentiality Here Read and Sign
- Take pet to Vets and get form filled out for FREE by the vets listed
- E-mail us using contact us form and let us know you are coming.
- Bring Volunteer, Pet and forms to orientation.
To be a Volunteer/pet team two things are mandatory, We must have a Pet Heath Certificate on your pet and you and your pet must attend a Volunteer/Pet team Orientation.
Training is requried for 1st time volunteers. This will be done on an as needed basis. Most likely once a month.
Our next training session is on August 24, 2008 from 9:00am to 12:00pm please see the calendar for more information! Before you and your pet can come to training we must have a Pet Health Certificate for your pet. Please Download a Copy at the above link.
Have your Vet fill out and then return to Unconditional Love, PO Box 5039, Hagatna, GU 96932, or bring it with you to the Volunteer/pet orientation. The Veterinarian's Listed below the form have donated their time to do the at no cost to the Volunteers, HOWEVER, this does NOT include lab fees, shots, medication or Surgery. If these are requied, the cost will be paid by the Volunteers.
